Instructions on how to use request access to a portal:
1. Submit a ticket to support@myquipt.com
2. Make a note of the ticket number in the automated response email
3. Open the form linked at the bottom of this page and fill out the fields as instructed in the prompts, being sure to input the ticket number from the automated email.
4. When you submit the form you will see a thank you screen, and you should receive an email shortly from JotForm with a copy of your request.
5. Verify the submitted form(s) had the correct ticket number, and the note was added to the ticket.

6. Once all your requests are completed reply to the ticket indicating this is the case, only after at that point will your request begin being processed.
Once step 6 is completed your request will be evaluated and processed.
